Healh insurance declaration
if you don't have health insurance valid in Poland for the duration of studies at the time of admission, you need to sign this declaration
Consent of the legal representative (underage)
if you're under the age of 18, your legal representative (parent, guardian) must consent to your participation in admission and any related processes
Power of attorney for delivery
in case you live outside of Poland, you must authorise somebody living in Poland to collect your documents of admission
Information on data processing
English-language exam declaration
in case you don't have an English proficiency certificate (level B2 or higher) or another document confirming your proficiency in that language, you need to take the English exam organised by the University
(PL) Polish-language exam declaration
